Kirby Bates Associates is excited to announce that it is working with Maine Coast Memorial Hospital to identify candidates for a Director of Care Coordination.    

The Organization

Maine Coast Memorial Hospital is a full service, 64 bed hospital located in Ellsworth, Maine serving Coastal Acadia and Downeast Maine. The comprehensive healthcare facility has been providing emergency, primary and specialty care, acute inpatient care, diagnostic services, and surgical services to Hancock and western Washington Counties since 1956.  In 2014, MCMH was ranked 4th among the top 25 hospitals in the nation for their individual scores on Value-Based Purchasing which measures clinical process of care, patient experience and outcomes. The organization was also named a “Top Performer on Key Quality Measures” by the Joint Commission. MCMH was also awarded a Grade A for patient safety by the Leapfrog Group, the healthcare industry’s independent quality reviewer.  Maine Coast recently signed a definitive agreement to join Eastern Maine Healthcare Systems (EMHS) and regulatory approval is anticipated later this year.


Maine Coast Memorial Hospital is seeking an experienced, progressive nursing leader to assume the position of Director of Care Coordination.  The Director will assume responsibility for care management, disease management, and inpatient and outpatient transition of care services to support safe, high quality patient care across the continuum. The Director reports directly to the Chief Nursing Officer/Vice President Patient Care Services. As a member of the senior leadership team, the Director will collaborate with colleagues to actively lead and contribute to the vision and goals of the organization and support the implementation of strategic organizational initiatives.

Qualifications and Personal Characteristics 

  • Licensee or eligible for licensure in Maine as a Registered Nurse.
  • Bachelor of Science in Nursing (BSN) or in healthcare related field preferred.
  • Certification in Case Management is highly preferred.
  • Three to five years of Care Management leadership experience required.
  • Contemporary knowledge of case management practice and standards of care, trends and evidence in the care of patients across the continuum.
  • Knowledge of contractual or risk arrangements.
  • Fiscal competency demonstrated by the development of annual budgets reflecting effective utilization of staffing, capital, and other operating resources.
