Wentworth-Douglass Hospital (WDH) is a not-forprofit charitable organization and is one of the New Hampshire Accountable Care Partners. WDH is affiliated with Wentworth Douglass Health Partners (Health Partners), a multi-specialty group practice to provide a coordinated system of care for services ranging from wellness to complex medical conditions.

The Director of Care Coordination (CC) reports to the Assistant Vice President, Inpatient Service and works closely with the Medical Director, Inpatient Services, the Manager, Utilization Review, and the Director of Clinical Practice Transformation at Health Partners. Responsible for over 23 FTEs, the Director will provide leadership to the team of Care Managers and Inpatient Social Workers.

The Director will bring contemporary knowledge of Care Management, Population Health and Value-based purchasing to develop a vision for the department. The Director will lead the staff to engage in interdisciplinary teams at the unit level to make decisions about care and create processes within the department to achieve excellent clinical results, and regulatory and financial goals. The team of inpatient and emergency service Case Managers and Social Workers are committed to their patient population and mission. The Director will develop and grow the team to manage effectively in a new model of care across the continuum, partnering with Health Partners.

The ideal candidate will have case management leadership expertise including an understanding of the payers, evidence-based standards for coordination and delivery of services to patients, and the ability to achieve desired outcomes. Candidates will have experience developing and coaching staff, and the ability to collaborate effectively with physician leaders and other disciplines. In addition, candidates will be skilled in negotiation, conflict management, and team building.

Candidates will have at least 5 to 7 years of progressive case management leadership with expertise in reimbursement methods, social work services and quality. A specialty Certification in Case Management is preferred. A Master’s Degree is required, along with New Hampshire RN license eligible.
